A paradigm shift in patient management, perioperative medicine is emerging as a holistic, system-wide strategy for encompassing care before, during, and after surgical interventions. This comprehensive approach is instrumental in mitigating complication rates, reducing hospital lengths of stay, yielding superior health outcomes, and enhancing overall health system efficacy, as detailed in a special publication featured in the Online First edition of Anesthesiology, the official peer-reviewed journal of the American Society of Anesthesiologists (ASA).
Perioperative medicine represents a more structured and integrated surgical process, wherein diverse specialties collaborate to amplify efficiency and bolster patient safety. Its vision entails surgical care as a cooperative endeavor involving surgeons, anesthesiologists, primary care physicians, nursing staff, and other allied health professionals, all working in concert to navigate the complexities of patient care throughout the entire surgical trajectory.

The scope of perioperative medicine encompasses pre-surgical evaluation and risk stratification, strategic interventions to optimize a patient’s physiological state prior to surgery, meticulous management within the operating theatre and during the recovery phase, and subsequent ongoing patient care and follow-up.
By championing interdisciplinary collaboration, effective leadership, clear communication, and the well-being of all involved, the perioperative medicine framework prioritizes improvements in quality of life, functional capacity, and the overall patient experience, extending beyond mere technical surgical accomplishment.
“From the perspective of patients and their families, this translates into more meaningful and timely preparation for surgical procedures, a reduction in procedure cancellations and postponements, enhanced clarity in communication among the multidisciplinary healthcare team, and a decrease in adverse complications – all of which have a direct bearing on recovery trajectories, healthcare expenditures, and an individual’s sense of well-being,” stated Thomas R. Vetter, M.D., the corresponding author of the journal publication and a member of the ASA’s Center for Perioperative Medicine. “By ensuring patients are in a healthier state pre-operatively, perioperative medicine offers a valuable opportunity to foster improvements in long-term health.”
This special article offers current insights and establishes consensus on advancements within the field, drawing from the 2024 and 2025 perioperative medicine stakeholder summits convened by the ASA’s Center for Perioperative Medicine (CPMed). The publication integrates practical observations from a wide array of healthcare professionals engaged in surgical care, encompassing both national and international medical communities.
The article situates perioperative medicine within the context of prevailing healthcare system challenges, such as staffing shortages, clinician burnout, and the imperative for accountability in patient outcomes. It underscores how perioperative medicine serves as a pragmatic solution for evolving payment methodologies, including episode-based and value-based models, such as the Centers for Medicare & Medicaid Services’ Transforming Episode Accountability Model (TEAM).
Evaluating the economic implications of perioperative medicine initiatives will be a pivotal step in the successful implementation of value-driven surgical care. “Healthcare systems stand to gain significantly from enhancements in care delivery processes and superior patient-reported outcomes,” remarked Dr. Cannesson. “These improvements position perioperative medicine as a strategic, system-level investment, necessitating dedicated governance, robust metrics, and financial alignment as an institutional priority.”
Building upon their collective discussions, participants at the CPMed perioperative medicine summit collaboratively devised a “forward-looking strategic plan” to delineate the future trajectory of perioperative care over the ensuing decade. The realization of successful endeavors will hinge on the synchronized integration of four key pillars: clinical care delivery, educational initiatives, research endeavors, and leadership development. “Advancements in any single area will not suffice,” asserted Dr. Cannesson. “Sustained progress requires the simultaneous cultivation of workforce training programs, standardized outcomes research, and effective organizational structures to champion multidisciplinary team collaboration.”
